Case Management

Why Would I Need Case Management?

The need for case management services is determined through a collaboration between you, your family or caregiver, your health care team, and your provider. This collaboration will include a comprehensive assessment of your health, psychosocial needs, and use of health care services and resources.

Case management can help by:
  • Coordinating your care
  • Assessing, planning, and facilitating services for you
  • Evaluating your options
  • Advocating on your behalf
You or your family member may need case management if you have:
  • Instability with chronic health problems
  • A serious terminal illness
  • An increased need for different provider specialties due to multiple diseases or conditions
  • A need for more support and education during a critical period

Who are Case Managers?

Case managers are usually nurses or social workers who can help you and your family figure out complex health care and support systems. They will work with you to coordinate the services and other community resources you need.

They can:
  • Provide advocacy, support, and education
  • Reduce burden and streamline appropriate utilization of care
  • Partner with members of your healthcare team to assist in coordination of your health care needs
  • Monitoring for progress and desired outcomes

Disease Management 

The Disease Management program uses evidence-based practice standards and current clinical practice guidelines to assist you with your chronic disease process and self-management goals. 
 
We aim to empower and improve your health and well-being by providing health coaching and education to assist you in reaching your wellness goals. 
 
Our program collaborates with you, your PCM, and the rest of your health care team to develop a joint plan of care to improve your understanding of your disease process and promote/maintain optimal health and wellness. 
 
The Disease Management Program is open to all beneficiaries. 

Who are Disease Managers? 

Disease managers are highly trained registered nurses who work in a specialty referral clinic.  We receive referrals from Primary and Specialty Care providers.  You or your family member may also self-refer. 

Why Would I Need Disease Management? 

Disease Management has the unique opportunity to provide you with a longer appointment time than normal clinic appointments, which allow you more of an opportunity to discuss your specific healthcare concerns and needs. 
 
Our disease managers seek opportunities to empower you to better understand your disease process, and prevention or minimizing long-term complications.  We can also discuss lifestyle interventions and medication management. 
 
We can assist with this by providing one-on-one appointments (in person or over the phone) and group classes that use a combination of education and motivation to reinforce self-management. 

What Disease Management Services are Available? 

We can assist you with the following conditions: 

  • Asthma 
  • Chronic Obstructive Pulmonary Disease 
  • Congestive Heart Failure 
  • Diabetes or Prediabetes 
  • Hypertension (High Blood Pressure) 
  • Hyperlipidemia (High Cholestenol) 
  • Weight management 

Utilization Management 

Utilization management nurses look at medical trends in health care services at both the military hospital and clinic and in the TRICARE Network. They assist your PCM team in identifying problems with access and services needed to ensure you receive the best health care available. 

Population Health 

Population Heath seeks to maintain a healthy, fit, and ready Air Force. Additionally, keeping family members and retirees in the best health possible is extremely important.
