Outpatient Medical Records / Release of Information

If a specialty provider requests medical documentation for a scheduled appointment, have the provider fax a Release of Information request to 623-856-8529. Please allow seven business days for processing unless a specific date is annotated on the request. Medical documentation received from off-base facilities pertaining to referrals to civilian physicians will be filed in your medical record.
