Family Health Clinic

The Family Health Clinic provides routine, preventive, and acute outpatient care for enrolled DOD beneficiaries. 

We aim to improve the health of our beneficiary population by providing safe, quality, evidence-based health care. FHC providers assess, manage, and coordinate acute and preventive health medical care, promote telehealth services, initiate specialty referrals/emergency transfers, perform minor procedures, and provide initial diagnosis, imaging, labs, and medication administration/refill. 
 
FHC manages chronic conditions such as, but not limited to: 

  • High blood pressure
  • Heart disease
  • Diabetes
  • High cholesterol
  • Asthma
  • Obesity
  • ​Cancer 

For conditions considered beyond the scope of the Family Health Clinic, patients will be referred to a specialist in the TRICARE network. 

Acute Appointments 

Acute appointments are for non-life-threatening illnesses that require patients to be seen by an FHC provider within 24 hours. Please call 623-856-CARE (2273) to request a same-day appointment, if available. Beneficiaries may utilize TRICARE network urgent care centers if acute appointments are unavailable. 

Routine Appointments 

Routine appointments are for present illnesses not causing severe symptoms. 

Routine appointments include: 

  • Annual exam with your provider
  • Hypertension
  • Thyroid issues
  • Mental health-related condition evaluation and follow up
  • Diabetes follow-up
  • Pap tests

Tele-Health 

FHC offers phone appointments to address routine issues such as: 

  • Medication clarification
  • Medication refills
  • Lab results
  • ​Referral renewals

Same Day Walk-in Appointments 

The FHC offers the following walk-in services for evaluation: 

  • Urinary tract infection concerns (females aged 18-65 only): Monday, Tuesday, Thursday, Friday, 9-11 a.m.
  • Vaginal discharge concerns (females aged 18-65 only): Monday, Tuesday, Thursday, Friday, 9-11 a.m.
  • Sore throat screening: Monday, Tuesday, Thursday, Friday, 9-11 a.m. and 1-3 p.m.
  • STI testing/confidence check: Monday, Tuesday, Thursday, Friday, 9-11 a.m. and 1-3 p.m.

Administrative Services 

Family and Medical Leave Act  & Disability Paperwork 

If you have a current diagnosis and need FMLA or disability paperwork completed for yourself or to care for a family member, you will need to make an appointment with your PCM. Please bring your paperwork to your appointment with all patient and employer information completed.   
 
If you are receiving specialty care, you will need to bring medical records to the appointment or provide records to FHC prior to your appointment, if records are not available in the MHS GENESIS Patient Portal

Exceptional Family Member Program Clearance Paperwork 

Patients needing EFMP clearance must first contact the MDG’s EFMP office at 623-856-4164/7296 for assessment of needs and guidance on appropriate forms (DD2792), then please bring your EFMP paperwork to your appointment with all patient information completed.  

Prepare for Your Appointment 

When you see your primary care manager, please prepare by: 

  • Bringing a current medication list
  • Letting your provider know what medication refills are needed
  • Bringing a blood sugar log if you are diabetic
  • Bringing a blood pressure log if you have hypertension (high blood pressure)
  • ​Making a prioritized list of any concerns or questions 

Please plan to arrive 15 minutes before your appointment time to complete necessary appointment intake paperwork and to mitigate any gate delays and unpredictable travel time, especially during inclement weather. While out of our patients’ control, delays up to 10 minutes can result in the loss of an appointment slot and the patient will need to be rescheduled. Please call 623-856-CARE (2273) as soon as possible if you are unable to make your scheduled appointment to cancel or reschedule.   

Prescription Renewals 

Please send a message to your PCM via the MHS GENESIS Patient Portal with medication renewal requests when you have no refills and two weeks of medication remaining. You may also opt to have your medications sent through Express Scripts home delivery.  

Prescription Refills 

To refill a prescription:  

  • Call the automated refill line at 623-343-1747 and select your preferred pickup location: Option 1 for Satellite Pharmacy, Option 2 for the Main Pharmacy, or Option 3 for the Script Center.
  • ​Navigate to the Rx Refills tab at the top of the MHS GENESIS Patient Portal.
